Call or E-mail HEX Dental Lab to receive a fee schedule, shipping labels and supplies to send a case. Download the Lab Prescription below.
Packaging a Case
Please package the impression or model with protective packaging materials to ensure safety. Bundle your cases; our boxes can easily fit 2-3 cases each for convenient shipping. Provide a detailed Lab Prescription Form regarding each case. Attach a prepaid shipping label and your cases are ready to go!
